Therapeutic Contract and Consent Details between Clients and Mithrah

The client agrees that the personal information about the clients and their therpauetic session will be saved in our website. The client also agrees that these information will also be saved and documented by their therapists in their system as well.

Any information exchanged during therapy between client and therpaists and or staff, which will put either client or the people known to clients at risk will be reported to relevant authorities. The client’s permission is not needed to do mandatory reporting.

The client gives consent to share the relevant information about their therapuetic goals and treatment plans among relevant team members of Mithrah and or therapists or other stakeholders.

client must ensure signing relevant consent forms and assessement forms before therapy and treatment begins.

The client can request Mithrah or the therapists to give the copy of the information regarding their therapy.

All information regarding the client will be shared with legal systems when they are subpoenaed by law.
